Need a Replacement Card? Order a Medicare Card by Phone or Online
If you lose or damage your Medicare card, you can obtain a replacement card from Social Security one of the following ways:
- Log into your MyMedicare.gov account. If you do not have a MyMedicare account, sign up now.
- Order Online by visiting the Medicare Card Replacement section of Social Security’s Web site. You card will be mailed within 30 days to the address SSA has on record.To make an online request, you will need the following information:
- Your last (exact) payment amount or the month and year you last received a payment if you have received benefits in the last 12 months.
- Your name as it appears on your most recent Social Security card
- Your Social Security Number
- Your Date of Birth
- Your phone number in case we need to contact you about your request
- Your e-mail address (optional)
You may also need:
- Your Place of Birth
- Your Mother’s Maiden Name (to help identify you)

The Centers for Medicare & Medicaid Services (CMS) administers Medicare, the nation’s largest health insurance program, which covers nearly 40 million Americans. Medicare is a Health Insurance Program for people age 65 or older, some disabled people under age 65, and people of all ages with End-Stage Renal Disease (permanent kidney failure treated with dialysis or a transplant).
Last night I applied for a replacement for my Medicare Card and was told that it would take 30 days b/4 I would get a replacement. This makes for a grave difficulty for me as it directly affects my retirement benefits that I am only a week away from. Is there a
“Temporary” version that I can get in lieu of getting the real thing?

Which office did you contact? If you have direct contact with a benefit admin look thru our website and find the local number. In addition most states have a web site link so you can print a temporary medicare card. Here is an example:
A Medicaid record is automatically created in the Medicaid Management Information System (MMIS) and in Ohio SACWIS when the child has been determined eligible for FCM or AA reimbursements under Title IV-E. An application can be recorded in Ohio SACWIS when the child is eligible for Medicaid benefits, like Healthy Start, for example, not related to Title IV-E. (See Record a Medicaid application.) In either case, information is sent to CRIS-E, which generates permanent Medicaid cards.
You can generate a temporary Medicaid card from Ohio SACWIS, for example, if the child needs a prescription filled quickly.
Generate a temporary Medicaid card as follows:
Click the Financial tab at the top of the screen.
Click the Eligibility tab.
Click the Medicaid Eligibility link in the menu on the left. The Child Selection search screen appears.
Click Person Search, then locate and select the child whose Medicaid eligibility you want to view. After you select the child, the screen displays histories of the Medicaid, HMO, and MMIS records.
Click the report link for the active Medicaid record in the MMIS Medicaid Eligibility History table.
Click Generate Report. The Document History screen appears. This screen displays a history of documents that were generated and saved on this case record.
Click Generate Report. The temporary Medicaid card appears in a PDF.
Print the PDF. The card is printed on your local network printer.
I need a new card. I lost mine.
To apply for a Social Security number and card:
* Complete an Application For A Social Security Card (Form SS-5); and
* Show us original documents or copies certified by the issuing agency proving:
o U.S. citizenship or immigration status [including Department of Homeland Security (DHS) permission to work in the United States];
o Age; and
o Identity.
Then, take or mail your completed application and documents to your local Social Security office.
My husband recently had a stroke, is not able to talk, he needs a replacement for his medicare card which was lost when he recently lost his wallet.
