REPLACEMENT MEDICARE CARD - www.ssa.gov/medicarecard
- As of July 11, 2000, the public can apply for replacement Medicare Cards on SSA’s Internet site.
- Applicants requesting a replacement card fill out the online form and send it electronically to SSA. It takes only a minute or two to complete.
- SSA uses the highest commercially available encryption to ensure that a beneficiary’s
confidential information is secure as it travels over the Internet. - Requests are forwarded each day to the Health Care Financing Administration, which processes the requests and mails the new card to beneficiaries.
- Since July 2000, more than 26,000 Medicare beneficiaries have used the service.

Original Medicare
Original Medicare is a fee-for-service plan managed by the Federal Government. In general, with Original Medicare:
* You use your red, white, and blue Medicare card when you get health care.
* You can go to any doctor or supplier that accepts Medicare and is accepting new Medicare patients, or to any hospital or other facility.
* You pay a set amount for your health care (a deductible) before Medicare pays its part. Then, Medicare pays its share, and you pay your share (your coinsurance or copayment) for covered services and supplies (unless you have a Medigap policy or other supplemental insurance that may pay for these costs.)
* You may have a Medigap policy or other supplemental coverage that may pay deductibles, coinsurance, or other costs that aren’t covered by Original Medicare.
How to Select a Medicare Health Plan
In Medicare, you can choose different ways to get the services covered by Medicare. Depending on where you live, you may have different choices.
1. In most cases, when you first get Medicare, you are in Original Medicare.
2. You may want to consider a Medicare Prescription Drug Plan to add prescription drug coverage.
3. You may want to consider a Medicare Advantage Plan (like an HMO or PPO) that provides all your Part A, Part B, and often Part D prescription drug coverage.
4. Or, if you choose Original Medicare, you may want to consider a Medicare Supplemental Policy (Medigap).
You make a choice when you are first eligible for Medicare. Each year you can review your health and prescription needs and switch to a different plan in the fall. There are things you should consider to help you meet your needs.

* What will you pay out-of-pocket, including premiums, deductibles, and other cost-sharing (copays or coinsurance)?
