I have just completed a divorce/settlement with my former married name…Stoddard…dropped and my MAIDEN NAME – JEGLIN was assigned back to me as of Nov. 13, 2009…How do I get a new Medicare Card made out to ANNA MARIE JEGLIN?

Special Instructions:All residents of Sacramento County must go to the Social Security Card Center to transact social security number-related business.

A mandatory interview must be conducted for all Social Security card applicants regardless of age.

The Sacramento Social Security Card Center does not provide mail-in service.
